Second Lieutenant Cyril Thornton, 28 Training Squadron Royal Flying Corps. Whilst flying Avro 504J, serial no B3129, the aircraft stalled in the air at Castle Bromwich, United Kingdom.
Court of Inquiry:
The court having considered the evidence are of opinion that an error in judgement in turning on the part of the pilot Lt Lewis caused the machine to spin and dive in to the ground which resulted in the death of both pilot and passenger.
