On 26th October 1941 HMS Tetrarch sailed from Malta bound for a refit in the U.S.A. Her route was via Gibraltar and the U.K. but she failed to arrive in Gibraltar on 2nd November and was declared overdue.  Her route passed through a known minefield and she is presumed lost on Italian mines off Capo Granditola, Sicily, Italy in late October 1941.

Thomas Jennings has no known grave, the photograph available shows his name on Portsmouth Naval Memorial.
