Samuel Evans enlisted in the Canadian Overseas Expeditionary Force on 15th January 1915 at Stratford, Ontario. On his attestation form he gave his date of birth as 5th November 1890 at Suckley, Worcestershire, England, his next of kin as his father, New Cottage, Suckley, Worcestershire and his occupation as a farmer. He was 5 feet 10½ inches tall with hazel eyes, dark brown hair and a medium complexion.

Samuel was posted overseas from England to join the 1st Battalion Canadian Infantry on 3rd August 1915. In October 1915 he was admitted to hospital in Etaples, France suffering from Pleurisy with further admissions to medical facilities for various wounds. Samuel was killed in action on 31st August 1916
