Sidney Phelps enlisted in the Canadian Overseas Expeditionary Force on 6th January 1916 at Winnipeg, Canada. On his attestation paper he gave his date of birth as 25th October 1897 at Chalford, near Stroud, Gloucestershire, England, his occupation as a Steam Fitter, his current address as 426 Victor Street, Winnipeg, Manitoba and his next of kin as his father, Andrew Phelps, Chalford, Gloucestershire. Sidney was 5 feet 10½ inches tall with a fresh complexion, grey eyes and brown hair.
Sidney sailed from St John, Canada on 11th March 1916, arriving in Liverpool, England on 25th March 1916. He embarked for France on 14th July 1916 arriving at Havre the following day.
Sidney was admitted first to No 9 Canadian Field Hospital on 5th December 1916 and on the same date to No 10 Canadian Field Ambulance suffering from conjunctivitis, he was discharged on 13th December 1916. On 2nd February 1917 Sidney Phelps was reported as seriously ill at No 2 General Hospital, Camiers, France, suffering from nephritis. He died from the illness the following day, 3rd February 1917. In 1920, his medals, scroll and plaque were sent to his mother, Annie Phelps, Worley Villas, France Lynch, Chalford, near Stroud, England.
